• Thank you for submitting your COVID-19 vaccination information. Please note that it may take up to 21 days for your submission to be assessed and uploaded to the Ontario COVID vaccine documentation system (COVAXon).
• Multiple submissions will be automatically rejected unless you are directed to re-submit by health unit staff.
• If you have submitted hand written COVID vaccine records you will be asked to provide further information and/or present in-person at the Agriplex clinic location.
• Once your record has been uploaded to the Ontario COVID vaccine documentation system, you will receive an e-mail notification with information on how to access your enhanced Ontario Vaccine Certificate/QR code through the Provincial COVID-19 Vaccination Portal https://covid19.ontariohealth.ca .
• Please note that you will need your date of birth, postal code and Ontario Health Card information to access your enhanced Ontario Vaccine Certificate/QR. If you do not have a green Ontario Health Card OR have any other issues call 226-289-3560.
• It is strongly recommended that you keep the original copies of your COVID-19 vaccine record for future reference.
• You are considered fully vaccinated in Ontario if they have received:
  • The full primary series of a COVID-19 vaccine authorized by Health Canada, or any combination of such vaccines (two doses of Moderna, Pfizer-BioNTech, Novavax, Medicago, AstraZeneca, including COVISHIELD) in any combination or one dose of Janssen (Johnson & Johnson); or
  • A full or partial primary series of a non-Health Canada authorized vaccine plus any additional recommended doses of a Health Canada authorized COVID-19 vaccine to complete the primary series; and
  • The final dose of the COVID-19 vaccine at least 14 days before providing the proof of being fully vaccinated
• If you require additional dose(s) of COVID-19 vaccine book an appointment with your health care provider, pharmacy or visit a community walk-in/pop up clinic.
