Submitting proof of COVID vaccination received out of Ontario/Canada

If you live in Middlesex-London and received one or more doses of the COVID-19 vaccine outside of Ontario or Canada, your record may be added to the provincial database. You must fall into the following categories to proceed:

  • You must live in Middlesex County or the City of London. You will need to provide your home address and this will be verified. If you do not live in the area, click here to find your local public health unit: local public health unit.
  • You must qualify under one of the following categories, or your doses will not be uploaded:
    • A child under the age of 5 years old
    • You received a dose within the last six months (all ages)
  • It is required that you upload a picture of the vaccine record. Please make sure that the picture is clear and the record includes your full name and date of birth.
  • All other doses will not be added to the provincial database at this time. Keep your original documentation in case you are required to show proof of your COVID-19 vaccine history for any purpose. Employers, educational institutions, healthcare providers and pharmacies do not need to have your doses in the system in order to assess your records. Your original record can be presented as proof of immunization.
