Before your appointment

Personal Support

Please only bring essential support persons with you when you come for your vaccination appointment. If you require assistance related to any mobility, visual, hearing or cognitive concern, your support person will be able to attend the appointment with you, and they will need to pass a COVID-19 screening questionnaire when you arrive at the vaccination clinic. They will not be able to receive the vaccine unless they are also eligible and have an appointment. A doctor’s note is not required in order to bring your support person. Due to COVID-19 guidelines enforced at the vaccination clinics (i.e. physical distancing and capacity limits), only individuals providing personal support should accompany you to your appointment .

Please note, the St. Thomas-Elgin Memorial Arena and Goff Hall – Woodstock Community Complex release new appointments each Monday for the following week. The Western Fair Agriplex, Caradoc Community Centre and North London Optimist Community Centre release new appointments daily.

A health card is not required to be vaccinated, but if you do have a health card, please use it when booking your appointment and bring it to your vaccination appointment.

1. Age-based criteria (65 years and over)

Please verify your eligibility by inputting your birthdate below. Proof of your date of birth will be required when you arrive for your appointment.


Only eligible dates can be selected. If you cannot select your birthdate it means you aren't eligible under this priority group.

2. Indigenous community

Indigenous adults 16 years of age and older. For those aged 16 & 17 you must book at Agriplex, St. Thomas Memorial Arena or Woodstock Goff Hall.

Please note, Indigenous vaccinators from SOAHAC are only available at the Western Fair District Agriplex, Monday to Friday between 9:00a.m. and 4:00p.m. (excluding holidays)


3. Eligible Occupations and Essential Caregivers

If you are a worker or essential caregiver in a sector that is currently eligible to receive the vaccine AND you have received a username and password from your employer or associated facility, please enter them below.

Essential caregivers are defined as individuals who provide regular and sustained assistance with personal care and/or activities of daily living for someone living in a long term care or retirement home, a congregate living setting or who has one of the highest risk health conditions and is currently eligible for vaccination.

4. Eligible Health Condition or Chronic Home Care or Congregate Living Setting Residents

If you are a chronic home care recipient or an individual with an eligible health condition or live in a high risk congregate setting AND you have received a username and password from your care provider, please enter them below.
