Before your appointment

Personal Support

Please only bring essential support persons with you when you come for your vaccination appointment. If you require assistance related to any mobility, visual, hearing or cognitive concern, your support person will be able to attend the appointment with you, and they will need to pass a COVID-19 screening questionnaire when you arrive at the vaccination clinic. They will not be able to receive the vaccine unless they are also eligible and have an appointment. A doctor’s note is not required in order to bring your support person. Due to COVID-19 guidelines enforced at the vaccination clinics (i.e. physical distancing and capacity limits), only individuals providing personal support should accompany you to your appointment .

Please note:

  • New appointments for St. Thomas- Elgin Memorial Arena, Goff Hall Woodstock, and Tillsonburg Community Centre are added each Monday.
  • New appointments open for Western Fair Agriplex, Caradoc Community Centre and North London Optimist Community Centre daily at 8:00 a.m.
  • Second-dose appointments cannot be booked online. If you need to book or make adjustments to your second-dose appointment, please call 226-289-3560 (8:00 a.m. to 7:30 p.m., Monday to Sunday)

A health card is not required to be vaccinated, but if you do have a health card, please use it when booking your appointment and bring it to your vaccination appointment.

1. Age-based criteria (40 years and over)

Please verify your eligibility by inputting your birthdate below. Proof of your date of birth will be required when you arrive for your appointment.


Only eligible dates can be selected. If you cannot select your birthdate it means you aren't eligible under this priority group.


2. Indigenous community

Indigenous adults 16 years of age and older.

Immediate household/family members of Indigenous people who themselves are not Indigenous.

Please note:
  • Indigenous vaccinators from SOAHAC are only available at the Western Fair District Agriplex, Monday to Friday between 9:00a.m. and 4:00p.m. (excluding holidays)
  • Indigenous adults age 16 & 17 must book at Western Fair Agriplex in London, North London Optimist Community Centre, St. Thomas Memorial Arena or Woodstock Goff Hall.

3. Eligible Occupations

Check the eligibility page to see if you work in an eligible occupation currently eligible to receive the COVID-19 vaccine.

Those aged 16 & 17 must book at Agriplex, North London Optimist Community Centre, St. Thomas Memorial Arena or Woodstock Goff Hall.

4. Chronic Home Care Recipient or Congregate Living Setting Residents

Check the eligibility page to see if you live in a high-risk congregate setting.

Those aged 16 & 17 must book at Agriplex, North London Optimist Community Centre, St. Thomas Memorial Arena or Woodstock Goff Hall.

5. Eligible Health Condition

Check the eligibility page to see if you have a health condition that is currently eligible to receive the COVID-19 vaccine.

6. Eligible Essential Caregiver

Certain essential caregivers are currently eligible to receive the COVID-19 vaccine.

Essential caregivers are defined as individuals who provide regular and sustained assistance with personal care and/or activities of daily living (e.g. feeding, bathing, dressing) for someone living in a long-term care or retirement home, a high-risk congregate living setting or who has a highest-risk or high-risk health condition.

7. Priority Postal Code

Applies to residents in postal code region N5H who are 18+.

To book an appointment, please select St Thomas Memorial, Woodstock Goff Hall or Tillsonburg Community Centre only.

You will need to provide proof of address when you arrive for your appointment (driver’s license, utility bill etc).
