COVID-19 Screening

You will be screened for COVID-19 at the door of the clinic and will not be allowed entry if you do not pass screening.

On the day of your appointment, ask yourself the questions listed below.
If you answer ‘Yes’ to any of the questions, please call 226-289-3560 to reschedule your appointment.

Are you experiencing any of these symptoms?
Fever, NEW or worsening cough, difficulty breathing/shortness of breath

Are you experiencing any of these UNEXPLAINED symptoms?
Fatigue/malaise, chills, sore throat, difficulty swallowing, runny nose, nasal congestion, digestive symptoms (including nausea, vomiting, diarrhea), loss of taste/smell, sore muscles, conjunctivitis (pink eye), headache

Have you travelled outside of Canada within the past 14 days?

If you’re healthcare worker:

  • Have you been in close contact* (without personal protective equipment) at home or in the community, OUTSIDE OF YOUR WORK, with someone who is SYMPTOMATIC and/or being tested/awaiting results of COVID-19?
    *A close contact includes someone you live with and/or a sick person with whom you spent more than 15 minutes outside of work, without any protection.

  • Have you been in close contact (without personal protective equipment) at home or in the community, OUTSIDE OF YOUR WORK, with someone who has been confirmed to have COVID-19 within the last 14 days?

If you’re not a healthcare worker:

  • Have you been in close contact* with someone who is SYMPTOMATIC and/or being tested/awaiting results of COVID-19?
    *A close contact includes someone you live with and/or a sick person with whom you spent more than 15 minutes while within two metres of each other.

  • Have you been in close contact with someone who has been confirmed to have COVID-19 within the last 14 days?
